New Patient Information
Welcome to Dermatique. To make your visit a positive experience and assure all necessary information is available to the doctor and staff, we ask you to review the following:
- Please read the Patient Privacy statement.
- Please print, read and complete the Patient Information forms and bring them to your first appointment at Dermatique.
- For patients who require referrals from their primary doctors, please make certain the referral is presented at the time of your visit to Dermatique.
- Please bring your medical insurance card.
- All co-payments and deductibles are due and payable at the time services are rendered. Dermatique accepts cash, checks, credit cards & debit cards.
